Nearly half of coronavirus patients admitted to Manhattans Mount Sinai Hospital developed acute kidney problems even though the vast majority had never had any issues with the organ before, according to a new report.
The find came in a study of nearly 4,000 pandemic patients to pass through the hospital between Feb. 24 and May 30, CNBC reported Monday.
Forty-six percent of patients developed some sort of kidney injury during their COVID-19 battles, the study found.
For 17 percent, the affliction was so severe that they required dialysis and more than a third of patients who beat the virus didnt regain the same kidney function they had before.
And most of the patients who developed problems 82 percent had never experienced any prior issues with their kidneys, according to the study.
Dr. Alan Kliger, co-chair of the American Society of Nephrologys COVID-19 Response Team, told the network that the finding is hardly limited to Mount Sinai.
What we have observed is that approximately 10 percent to 50 percent of patients with severe COVID-19 that go into intensive care have kidney failure that requires some form of dialysis, Kliger told CNBC.
Theres no one explanation for exactly how the virus decimates the kidneys.
Some biopsies have found that the virus directly affects the organ, Kliger said.
Other evidence shows that COVID-19 can trigger a cytokine storm an overreaction by the immune system that hurts ones kidneys and other vital organs in the process of trying to fight off the contagion.

In some extreme cases, the coronavirus can trigger sepsis, and with it, multiple organ failure, according to Kliger.
There is also evidence that life-saving ventilators can have the unintended side effect of restricting blood flow through the kidneys, potentially damaging them even as they see patients through the virus respiratory attacks.
Dr. Steven Coca, associate professor of nephrology at Mount Sinai Health System, told CNBC that once the coronavirus is defeated, the next battle will be against its lingering effects on survivors kidneys.
Since the start of the coronavirus pandemic, we have seen the highest rate of kidney failure in our lifetimes, he told the network. The next epidemic will be chronic kidney disease in the US among those who recovered from the coronavirus.

The virus causing the COVID-19 pandemic, SARS-CoV-2, presents at least six strains. Despite its mutations, the virus shows little variability, and this is good news for the researchers working on a viable vaccine.
These are the results of the most extensive study ever carried out on SARS-CoV-2 sequencing. Researchers at the University of Bologna drew from the analysis of 48,635 coronavirus genomes, which were isolated by researchers in labs all over the world. This study was published in the journal Frontiers in Microbiology. It was then possible for researchers to map the spread and the mutations of the virus during its journey to all continents.
The first results are encouraging. The coronavirus presents little variability, approximately seven mutations per sample. Common influenza has a variability rate that is more than double.
"The SARS-CoV-2 coronavirus is presumably already optimized to affect human beings, and this explains its low evolutionary change," explains Federico Giorgi, a researcher at Unibo and coordinator of the study. "This means that the treatments we are developing, including a vaccine, might be effective against all the virus strains."
Currently, there are six strains of coronavirus. The original one is the L strain, that appeared in Wuhan in December 2019. Its first mutationthe S strainappeared at the beginning of 2020, while, since mid-January 2020, we have had strains V and G. To date strain G is the most widespread: it mutated into strains GR and GH at the end of February 2020.
"Strain G and its related strains GR and GH are by far the most widespread, representing 74% of all gene sequences we analyzed," says Giorgi. "They present four mutations, two of which are able to change the sequence of the RNA polymerase and Spike proteins of the virus. This characteristic probably facilitates the spread of the virus."
If we look at the coronavirus map, we can see that strains G and GR are the most frequent across Europe and Italy. According to the available data, GH strain seems close to non-existence in Italy, while it occurs more frequently in France and Germany. This seems to confirm the effectiveness of last months' containment methods.
In North America, the most widespread strain is GH, while in South America we find the GR strain more frequently. In Asia, where the Wuhan L strain initially appeared, the spread of strains G, GH and GR is increasing. These strains landed in Asia only at the beginning of March, more than a month after their spread in Europe.
Globally, strains G, GH and GR are constantly increasing. Strain S can be found in some restricted areas in the U.S. and Spain. The L and V strains are gradually disappearing.
Besides these six main coronavirus strains, researchers identified some infrequent mutations, that, at the moment, are not worrying but should nevertheless be monitored.
"Rare genomic mutations are less than 1% of all sequenced genomes," confirms Giorgi. "However, it is fundamental that we study and analyze them so that we can identify their function and monitor their spread. All countries should contribute to the cause by giving access to data about the virus genome sequences."

Geographic and Genomic Distribution of SARS-CoV-2 Mutations

The novel respiratory disease COVID-19 has reached the status of worldwide pandemic and large efforts are currently being undertaken in molecularly characterizing the virus causing it, SARS-CoV-2. The genomic variability of SARS-CoV-2 specimens scattered across the globe can underly geographically specific etiological effects. In the present study, we gather the 48,635 SARS-CoV-2 complete genomes currently available thanks to the collection endeavor of the GISAID consortium and thousands of contributing laboratories. We analyzed and annotated all SARS-CoV-2 mutations compared with the reference Wuhan genome NC_045512.2, observing an average of 7.23 mutations per sample. Our analysis shows the prevalence of single nucleotide transitions as the major mutational type across the world. There exist at least three clades characterized by geographic and genomic specificity. In particular, clade G, prevalent in Europe, carries a D614G mutation in the Spike protein, which is responsible for the initial interaction of the virus with the host human cell. Our analysis may facilitate custom-designed antiviral strategies based on the molecular specificities of SARS-CoV-2 in different patients and geographical locations.

Introduction

Initially reported in mid-December 2019 in the Chinese city of Wuhan, the newly emerged severe acute respiratory syndrome virus (SARS-CoV-2) is a single-stranded RNA beta-coronavirus with a compact 29,903 nucleotides-long genome. This virus causes a serious disease known as Coronavirus Disease 2019 (COVID-19), which has spread in over 210 countries in <4 months, counting more than 10 million confirmed cases and almost 500,000 deaths reported worldwide as of June 28, 2020 (source: World Health Organization). A difference in case fatality rates across countries was observed, possibly due to a diverse demographic composition and the type of measures that have been taken in different countries to limit viral spreading (Dowd et al., 2020). According to data from the public database of the Global Initiative on Sharing All Influenza Data (GISAID), three major clades of SARS-CoV-2 can be identified (Forster et al., 2020), that have been subsequently named as clade G (variant of the spike protein S-D614G), clade V (variant of the ORF3a coding protein NS3-G251), and clade S (variant ORF8-L84S). However, as more complete sequences become available, the need to define specific geographic distributions of virus variants becomes of practical importance to define clinical and political strategies at the local level. Despite several reports having confirmed a relatively low variability of SARS-CoV-2 genomes (Ceraolo and Giorgi, 2020Lu et al., 2020), it is still unclear if different fatality rates or speed of transmission observed in different countries may be the consequence of clade's differences in virulence, as discussed by a recent commentary comparing different strains in the USA (Brufsky, 2020). It is therefore possible that more insights into the pathogenesis and virulence of this virus may come from comparative genomic analysis linked to epidemiologic data coming from different countries.
Genetic variance analyses must now play a crucial role in expanding knowledge on this new virus to adopt measures to contain its outbreak. Complete viral genome sequences have been made rapidly publicly available to the research community and have recently surpassed the 48,000 units, thanks to the worldwide effort of scientists and to the GISAID consortium. This data avalanche will result in an unprecedently rapid effort to analyze data to understand genome diversity (Andersen et al., 2020Shen et al., 2020), to hypothesize suitable targets for drug repositioning (Wu et al., 2020Zhou et al., 2020) and to develop prevention strategies (Zhao and Chen, 2020). In the present study, we performed the largest comparative study so far by analyzing more than 48,000 complete SARS-CoV-2 genomes. We report all mutations and stratify them genomically and geographically, also highlighting insurgence of sub-clades and genomic highly variable spots. These finding may be extremely useful to design and think about the efficacy of measures that have been taken on a regional basis to limit SARS-CoV-2 spreading.

Methods

Forty-eight thousand six hundred thirty-five SARS-CoV-2 genomic sequences were downloaded from GISAID (Shu and McCauley, 2017) on June 26, 2020 (Supplementary File 1). Only viruses affecting human hosts were selected, removing low-quality sequences (>5% NNNs) and using only full-length sequences (>29,000 nt). Forty-eight thousand six hundred twenty-four sequences were associated to a geographic region, specifically: 514 from Africa, 3,340 from Asia, 31,818 from Europe, 10,250 from North America, 2,127 from Oceania and 575 from South America. Eleven sequences were not associated to any continent. We provide as Supplementary File 2 a full geographic description of each sample used in the study.
The reference NC_045512.2 SARS-CoV-2 Wuhan genome (Coronaviridae Study Group of the International Committee on Taxonomy of Viruses, 2020), 29,903 nucleotides long, was obtained from NCBI GenBank. A GFF3 annotation associated to the refence, showing genomic coordinates for all protein sequences of SARS-CoV-2, is provided as Supplementary File 3. The large ORF1 polyprotein was split into its constituent Non-structural proteins (NSPs). The NSP12, encoding for the viral RNA-dependent RNA polymerase, was considered in the annotation as two regions, NSP12a and NSP12b, corresponding to the regions before and after a ribosomal frameshift, occurring as nucleotide 13,468 is translated as both the last nucleotide of a codon and the first of the next codon.
NUCMER version 3.1 (Delcher, 2002) was used to align all 48,635 genome sequences over the NC_045512.2 reference. The output of the alignment was converted to an annotated list of all mutational events using an internally developed R SARS-CoV-2 annotation algorithm provided as Supplementary File 4.
SARS-CoV-2 5UTR RNA secondary structure has been predicted by free energy minimization together with equilibrium partition function and base pair binding probabilities algorithm from the RNAfold WebServer using default settings (Gruber et al., 2008).

Results

Our analysis of 48,635 SARS-CoV-2 highlights a total of 353,341 mutation events compared to the NC_045512.2 Wuhan reference genome. Our results, event by event, are available as Supplementary File 5. While 256 samples, mostly originating from Asia, did not have any difference from the reference, 48,379 samples possessed at least one mutation. The number of mutations is relatively low, with mode per sample equaling 6, an average of 7.23, and very few samples having more than 15 events (Figure 1A). Overall, no continent differs significantly from the average mutation rate (Figure 1B), but there is a significant difference (one-way ANOVA p = 9.55 × 10205) in the average number of mutations per sample between countries. Specifically, amongst the top 40 nations with the highest number of sequenced full viral genomes (Figure 1C), these countries have a slightly but significant higher number of observed mutations per sample, when compared to the world's average: India: (8.40), Congo (8.30), Bangladesh (9.83), and Kazakhstan (9.47). On the other hand, the sequences from the following countries show a significantly lower mutational burden: Germany (6.09), Japan (4.55), Italy (5.92), Greece (5.91), Hong Kong (5.00), and Kenya (5.38). One must bear in mind that some sampling biases may affect this comparison: for example, some countries have generated the highest number of sequences in the early phases of the pandemic, and may have therefore observed fewer mutations (for example, Italy has not shared any sequence in the months of May and June 2020, the last two considered in our analysis). On the other hand, one would expect China to have a lower number of mutations, being the likely point of origin of SARS-CoV-2 (Ceraolo and Giorgi, 2020), and indeed the distribution of mutations per sample seems to suggest that (Figure 1C); however, a small number of sequences carrying a very high number (>50) of mutations are associated to China, shifting the distribution for this country. Upon manual inspection, these sequences do not appear to share similarities between each other, and are likely the product of technical sequencing errors.

The World Health Organization has completed groundwork in China for its probe into the origins of the coronavirus as the agencys chief warned Monday there may never be a silver bullet to the deadly disease.
The WHO sent two experts an animal health expert and an epidemiologist to the country early last month to investigate how the virus jumped from animal to human.
A larger, WHO-led team, made up of Chinese and international experts, will now begin studying the origins of the virus in Wuhan, where it was originally believed to have come from a wet market.
Epidemiological studies will begin in Wuhan to identify the potential source of infection of the early cases, WHO Director-General Tedros Adhanom Ghebreyesus said at a virtual press conference Monday.
Tedros touted the race to develop a COVID-19 vaccine as unprecedented but warned that the virus may be around forever.

More than 18 million cases of coronavirus have been confirmed across the globe, with 689,625 deaths, according to data from Johns Hopkins University.

Minks and COVID-19: Scientists study coronavirus outbreaks among animals in Spain, Netherlands

MADRID -- Coronavirus outbreaks at mink farms in Spain and the Netherlands have scientists digging into how the animals got infected and if they can spread it to people.
In the meantime, authorities have killed more than 1 million minks at breeding farms in both countries as a precaution.

The virus that first infected people in China late last year came from an animal source, probably bats, and later spread from person to person, as other coronaviruses had done in the past. Some animals, including cats, tigers and dogs, have picked up the new coronavirus from people, but there hasn't been a documented case of animals spreading it back to humans.
The outbreaks among the minks on farms in the Netherlands and Spain likely started with infected workers, although officials aren't certain. But it also is "plausible" that some workers later caught the virus back from the minks, the Dutch government and a researcher said, and scientists are exploring whether that was the case and how much of a threat such a spread might be.

The outbreak at the Spanish mink farm near La Puebla de Valverde, a village of 500 people, was discovered after seven of the 14 employees, including the owner, tested positive in late May, said Joaquín Olona, regional chief of agriculture and environment. Two other employees got infected even after the operation was shut down.
More than 92,000 minks were ordered killed at the farm in the Aragon region of northeastern Spain, with nine out of 10 animals estimated to have contracted the virus.
After the Dutch outbreaks began in April, professor Wim van der Poel, a veterinarian who studies viruses at Wageningen University and Research, determined that the virus strain in the animals was similar to the one circulating among humans.
"We assumed it was possible that it would be transmitted back to people again," the virus expert said, and that's what appeared to have happened with at least two of the infected workers.

Richard Ostfeld, a researcher at the Cary Institute of Ecosystem Studies in Millbrook, New York, said that if confirmed, these would be the first known instances of animal-to-human transmission.
"With the evidence for farmed mink-to-human transmission, we definitely need to be concerned with the potential for domesticated animals that are infected to pass on their infection to us," Ostfeld said by email.

The U.S. Centers for Disease Control and Prevention says some coronaviruses that infect animals can be spread to humans and then spread between people, but it adds that this is rare.
Both the World Health Organization and the Paris-based World Organisation for Animal Health are studying the transmission of the virus between animals and people. Several universities and research institutes also are examining the issue.
The WHO has noted that the transmission on the mink breeding farms could have happened both ways. But WHO's Dr. Maria Van Kerkhove said at a news conference last month that such transmission was "very limited."
"This gives us some clues about which animals may be susceptible to infection and this will help us as we learn more about the potential animal reservoir of (the virus)," she said, referring to cases in the Netherlands and Denmark, another major producer of mink fur.

While scientists think the virus originated in bats, it may have passed through another animal before infecting people. A WHO team is currently in China, planning to study the issue.
More than 1.1 million minks have been killed on 26 Dutch farms that recorded outbreaks, according to the Netherlands Food and Consumer Product Safety Authority. The government announced Thursday that minks at a 27th farm also were infected and would be killed.
The Netherlands, which has some 160 mink farms, is the world's fourth-biggest producer of the prized fur after Denmark, China and Poland, according to Wim Verhagen, director of the Dutch federation of fur farmers. Spain has 38 active mink breeding operations, most of them in northwestern Galicia.
Both Spain and the Netherlands have tightened hygiene protocols at mink farms and banned transportation of the animals and visits to the buildings where they are kept.
China, which produces about a third of the mink fur market, and the United States have not reported any virus outbreaks in minks or in animals at other farms.

Is this, at least in some part, a reflection of the continuous, deliberate, intentional spread? Are the travel waves superimposed on the pre-existing "sustained community transmissions"? Is this more than just one infection with Sars-Cov-2? Given the complex and polymorphous clinical picture and symptoms more consistent with the Multi-system Inflammatory Syndrome, including in children, rather than just the "Atypical Pneumonia" addressed in early Chinese studies, this question is quite legitimate. Hypothetically, the co-infections can include the Hantaviruses and the African Swine Fever, among the others. 

For how long did the hypothetical local reservoirs of infection(s) exist, and how were they formed? Was this in any way intentional or facilitating also? What was the role of the various animals, including pets and livestock in formation of these hypothetical reservoirs? 


What is the role of rodents? Is the unusual proliferation of the city rats, including the unusually large (Well fed ones? Nutrias, contaminating the waters?) worldwide and their unusual aggressive and "crasy" behaviors a sign of epizootics in rodents? They were quite often the vectors in the epidemics throughout the history, and probably for millennia. 


The infections in other, biologically close to them animals, such as minks, are very well documented and reported. Are these epizootics "natural" or induced, set intentionally? The political interests  and agendas are clearly visible in the fur farming affairs. 


What drives this Pandemic and the local epidemics, what are their mechanisms? About the half of all transmissions worldwide are of the origins which are described as "unknown", "undetermined", "cryptic", "multiple" and "independent". And this "unknown" half is probably the source of the "known", traced down half. So essentially, the sources and the origins of this Pandemic are largely, if not entirely,  UNKNOWN.

Union Home Minister Amit Shah, Karnataka Chief Minister B S Yediyurappa and Tamil Nadu Governor Banwarilal Purohit have tested positive for coronavirus, the latest among prominent public figures in the country to have contracted COVID-19, while Uttar Pradesh minister Kamal Rani Verma succumbed to the infection on Sunday. The chief of BJP's Uttar Pradesh unit, Swatantra Dev Singh, and state minister Mahendra Singh have also tested positive for COVID-19.
Taking to Twitter, 55-year-old Shah said he had undergone a coronavirus test after showing initial symptoms. My health is fine but I am getting admitted to a hospital on the advice of doctors," he tweeted in Hindi and requested those who had come in contact with him in the last few days to get tested for coronavirus and isolate themselves.
Shah has been actively leading efforts to fight the pandemic in the country where the case tally crossed the 17 lakh mark on Sunday. Political leaders, including BJP president J P Nadda, Rahul Gandhi of the Congress, DMK chief M K Stalin, Delhi Chief Minister Arvind Kejriwal and West Bengal Chief Minister Mamata Banerjee, wished Shah a speedy recovery.
In Uttar Pradesh, Kamal Rani Varun, the only woman cabinet minister in the state, died of COVID-19 at a hospital in Lucknow on Sunday. She was 62. The Technical Education minister, who tested positive for COVID-19 on July 18, breathed her last at the Sanjay Gandhi Postgraduate Institute of Medical Sciences (SGPIMS).
She had comorbidities, including diabetes, hypertension and hyperthyroidism, a senior doctor at the hospital said. President Ram Nath Kovind, Prime Minister Narendra Modi, Chief Minister Yogi Adityanath and other leaders expressed grief over her demise.
Uttar Pradesh BJP unit chief Swatantra Dev Singh, said on Twitter that he has tested positive for COVID-19 and has quarantined himself at his home following doctors' advice. Officials said that UP Jal Shakti Minister Mahendra Singh tested positive for COVID-19 around two-three days ago and has been hospitalised.
Earlier, state Health Minister Jai Pratap Singh, Rural Development Minister Rajendra Pratap Singh alias Moti Singh, Sainik Kalyan Minister Chetan Chauhan, Ayush Minister Dharam Singh Saini and Youth Development Minister Upendra Tiwari had tested positive for COVID-19. On Sunday night, Karnataka Chief Minister Yediyurappa tweeted that he has tested positive for coronavirus and is being hospitalised.
"Whilst I am fine, I am being hospitalised as a precaution on the recommendation of doctors. I request those who have come in contact with me recently to be observant and exercise self quarantine," he said. Yediyurappa is the fourth member of his cabinet to contract the viral disease. Earlier, Forest Minister Anand Singh, Tourism Minister C T Ravi and Agriculture Minister B C Patil had tested positive for COVID-19.
According to a bulletin issued by the Kauvery Hospital in Chennai, Tamil Nadu Governor Banwarilal Purohit has tested positive for COVID-19, and has been advised home isolation since his infection was mild. Purohit, 80, was in self-isolation since July 29 after three people in the Raj Bhavan tested positive for the coronavirus.
The bulletin said the governor underwent further tests on Sunday. He has been advised home isolation and a medical team would monitor him. Earlier, as many as 84 security and fire services personnel deployed in the Raj Bhavan tested positive for the virus on July 23, but the Governor's office had then said none of them came into contact with Purohit or senior officials.
Several MLAs in the state had earlier tested positive for COVID-19, while DMK legislator J Anbazhagan had died in June. Last week, Madhya Pradesh Chief Minister Shivraj Singh Chouhan had tested positive. Some state ministers and BJP leaders who met him had quarantined themselves at their homes Chouhan, 61, was admitted to a COVID-19 designated private hospital in Bhopal.
In West Bengal, Trinamool MLA Tamonash Ghosh, 60, had passed away in June due to the coronavirus. Meanwhile, megastar Amitabh Bachchan on Sunday said that he has tested negative for COVID-19 and will now be quarantining at home.
The 77-year-old actor, who was admitted to Nanavati hospital in Mumbai along with son Abhishek after testing positive for COVID-19 on July 11, expressed his gratitude to well-wishers for their continued support and prayers. Abhishek Bachchan, 44, however, said he is still COVID-19 positive and will remain under medical care.
According to the Union Home Ministry data, the total coronavirus cases surged to 17,50,723, on Sunday while the death toll climbed to 37,364. The recoveries have increased to 11,45,629, while there are 5,67,730 active cases of coronavirus infection in the country.
The recovery rate among COVID-19 patients has risen to 65.44 per cent while the fatality rate has further dropped to 2.13 per cent.
